The children's hands are not being washed in between diaper changes before interacting with the other children.

It was found that a parent updated the child's enrollment record by requesting a person be added as a designated pick up person, but the enrollment record was not physically updated and a signature of the parent was not obtained to verify the update.

Based on information obtained in an investigation the operation did not have a policy in place to record and verify the identify of an individual that picked up a child in care.

During the inspection it was found that 1 child did not have emergency medical authorization information.

During the inspection it was found that more than 12 months had passed since the last Child Product Safety document was certified and signed by the director.

At the time of inspection, the director did not have required transportation training available for review.

At the time of inspection the outdoor activity areas were observed with litter and overgrown grass/weeds. There was also a border along the fence with sections of it coming out of the ground posing a tripping risk. There was also a broken vent observed outside of the building in the Toddler area accessible to children and a large vine growing outside of the door with spikes which was also accessible to children. Additionally in the toddler outdoor area, there was an open sprinkler control panel with exposed wires accessible to children.

The center had a 35 month old child combined with four month old infants.

The filed trip posting did not list when the children were expected to return to the child-care center.

The infant and toddler rooms were missing electrical outlet covers. The director installed the safety covers and corrected the deficiency.

I observed tires on the playground that did not have drainage holes.

A ripped changing mat was observed in the toddler room.

The operation was operating over capacity, The operation is licensed for 51 children and during the inspection 54 children were present.

A shelf tipped over a child and an incident report was not provided to the parent.

Toddlers cups were not labeled.

Shelves were not anchored to the walls, as a result a shelf tipped over a child.
