An operation violated a condition when a caregiver with a provisional was alone in the toddler classroom during nap time. This was corrected when the director added a second classroom teacher.

In the young toddler classroom there were 10 children with a specified age group of 18 to 23 months with one caregiver which placed the classroom over ratio by one child. NOTE: This was addressed at inspection when one child was moved into the two year old classroom.

A open bag of garden soil was observed sitting on a low table accessible to children. This was corrected during inspection when the caregiver was observed removing the bag of soil from the classroom and making it inaccessible to children by placing it in a storage area.

Two sippy cups were observed not properly labeled with the child's first name and the first initial of the last name. This was corrected during inspection when the caregiver was observed labeling the sippy cups with the appropriate required information.

Notarized affidavit was missing on file for one staff record. This was corrected during inspection.

Director's qualification expired 5/20/2021. Director stated that she has submitted all her paper work to her training organization and at the moment waiting for her Certificate.
