Based on information gathered, it was determined that all bottles were labeled; however, a caregiver failed to ensure that the correct bottle was given to the right child, resulting in an infant being fed another child's mother's breastmilk.

Caregivers were observed diapering and sweeping the floor while children were climbing into and on top of furniture. Biting incidents reported have occurred when children have become frustrated during conflicts over toys.

Two children exited the classroom to a playroom across the hallway. The playroom did not have a caregiver present. One child was unattended for approximately 70 seconds, and the second child was unattended for approximately 1 minute and 70 seconds.

Parent notification regarding children exiting the classroom unattended was not reported to the parents until two days after the operation was made aware of the incident.

While reviewing one caregiver's annual training certificates, it was determined that the caregiver completed 46 hours of training in one day.

One out of ten child files evaluated was missing updated vaccination records.

A caregiver who did not meet the requirements was left in charge of children on three different occasions without having a qualified caregiver present.

The operational policies did not include information regarding parent rights.

One child's medication kept at the operation expired 12/2023.

The only caregiver in the classroom, at the time a child sustained a minor injury, was not first aid certified.
