A baby walker observed being used in the infant room. CORRECTED when baby walker was removed from room.

Upon arrival, two infant-age children were combined with children ages 2-3yo in the Toddler 2 room.

Children's Product Certification form was last certified on 03/01/2024. Note: This was corrected at inspection when provider completed the certification.

Last fire drill was conducted on 03/25/2024 and it was completed in 5 minutes. Last CO detector / fire extinguisher checks were completed on 03/25/2024. Note: CO detector and Fire extinguisher were checked during inspection and checks were documented under Emergency Practices Form.

Eight staff files were reviewed. One file is missing copy of Driver's License, a second file is missing Pre-Service training and a third one has an expired First Aid / CPR Training.

Some of the mats in the older infants, toddler and prek rooms were observed to be torn. Mats in the older infant room are not marked so that the sleeping side can be distinguished from the floor side.

Napping mats in the prek room were not individually assigned nor had a side designated to side to differentiate the napping from the floor side.

There was a bucket filled with water in the operation's playground. Note: This was corrected at inspection when provided emptied the bucket.

Operation has lesson plans posted, however, children were not observed participating in the activities listed under the plan nor following the daily schedule.

Ants were observed in the sink of the infant classroom and on furniture in the school-age room. AC vents in all suites have dust buildup. A side of the fence is slanted. Diaper changing mats in both infant room were torn. Broken toys and a broken cabinet were observed out in the playground. Note: During inspection provided replaced the diaper mats and removed the broken cabinet and toys.

One caregiver was supervising a group of 9 children ages 7mo to 27mo, specified age group was 12-17mo. Operation was over ratio by 4 children and age difference between the youngest and oldest child exceeded 18months.

Two out of eight children's files reviewed are missing immunization records.

An infant age-child was observed in a crib awake inside a crib for 31 minutes. Note: Child was removed from crib, a rug was brought to the classroom to allow child to move freely around the room.

Staff was not familiar with the ages of four children napping in the multi-purpose room. Staff supervising the toddler and prek children did not know how many children were in care.

One caregiver was supervising a group of eight napping children ages 12mo, 12mo, 14mo, 15mo, 18mo, 18mo, 18mo and 19mo.

One staff member was caring for five children ages 6 weeks, 7mo, 9mo, 11mo and 14mo. Classroom was over ratio by one child.

A toddler age child was observed unsupervised outside the child's classroom.

Two staff files are missing 1-7 hours of pre-service training. A third file is missing copy of ID, employment date and high school diploma/GED. A fourth file is missing orientation statement.

One of out six children's records reviewed is missing medical care information, permission to transport, parent/child address, emergency contact address and special care statement.

Outdoor fence had two loose planks of wood; one of them had nails sticking out. Another area of the fence had three protruding screws. There is a TV in the prek room and another one in the school-age room. Both TV are on top of cabinets and not anchored nor mounted. Note: Fence was fixed during inspection when provider nailed the nails and screws.

Six staff training record were reviewed. One staff members CPR training expired on 3/31/2022. A second file is missing trainings on transportation, abuse/neglect, safe sleep / shaken baby/ early brain development, hazardous materials, food allergies, medications, emergency preparedness, building and physical premises safety and communicable diseases. A third file is missing trainings on hazardous materials, food allergies, medications, emergency preparedness, building and physical premises safety and communicable diseases.

Written lesson plans and daily schedules were observed posted in all classsrooms. However some of the caregivers were not aware of the planned activitties and schedule was not being followed. One of the of five groups of children present was observed participating in the planned activity listed in the lesson plan. Infant children do not participate in outdoor play and a child was observed using a tablet to play a game.

The todddler and multi-purpose rooms are regularly used for prek age children. These rooms do not have well defined interest centers.

Fifty children ages 8mo to 11yo were combined in the school-age classroom for a party. Party lasted over an hour.

Fifty children were combined in the schooll-age classroom. School-Age classroom is licensed for twenty one children.

Based on the information obtained during the course of the investigation, a staff member encouraged a child to hit a child back.

Based on the information obtained during the course of the investigation, operation was over capacity by up to 11 children for four days between the hours of 3:30 - 6:15PM.

Binder taken during transportation routes is missing emergency medical transport and treatment authorization forms for each child being transported.

There is no space in between three cribs in the infant room. Note: This was corrected at inspection when cribs were re-arranged.

A caregiver was observed changing three children's diapers; in between changes caregiver nor children washed hands.

There are two slides with a height of 22 inches without the required surfacing. One slide is in the infant room and the second one in the school-age room. Note: This was corrected at inspection when slides were removed from operation.

One out of four staff files reviewed has a high school diploma/ged from a foreign country that has not been translated into english.

An employee was present at the operation prior to operation receiving background check eligibility notice.

Operation does not have a transportation emergency plan.

There is not adequate storage space for each child s personal belongings in the school-age classroom.

Operation has no mats or cots for children to nap on.
