It was determined that a caregiver slapped a child in the face.

Staff files were observed missing the following; - One staff is missing training in emergency preparedness; -Six staff are missing training in preventing and controlling the spread of communicable diseases, -Three staff are missing training in administering medication, -Six staff are missing training in preventing and responding to emergencies due to food or an allergic reaction, -Six staff are missing training in understanding building and physical premises safety, and -Six staff are missing training in handling, storing, and disposing of hazardous materials

Staff are missing the following trainings; -Four staff are missing recognizing and preventing shaken baby syndrome and abusive head trauma, -Two staff are missing understanding and using safe sleep practices and preventing sudden infant death syndrome and -Four staff are missing understanding early childhood brain development.

A caregiver did not properly position themselves on the playground when a child was able to open the emergency gate and get out. The caregiver was unaware the child was outside of the gate.

A parent was not notified immediately of a situation involving their child.

Eight staff files were observed and missing the following; Two staff are missing a signed statement that they received the center policies. Two staff are missing a signed statement that they received the personnel policies. Two staff are missing signed statements for completing orientation.

Ten children files were observed and missing the following; Five children files are missing the special care statements. Three children files are missing the parents signing off on receiving the parent handbook. Three children files are missing health statements.
