During the investigation it was determined a child was sent home with a wet diaper and had to be brought back in to be changed.

During the investigation it was determined that children have been injured when caregivers did not intervene in the activity that caused the injury. Groups of children were also observed, during inspection, to be running through the building, from one class area to another without structure of the caregiver being able to see where the children are or what they are doing. The management of children's behaviors are not under control.

During inspection 2 of four personnel was missing annual training documentation.

A classroom did not have enough light so my eyes did not have to adjust to see inside the room.

During inspection 4 files were reviewed and all four were missing required information.

During the investigation, it was found that a caregiver pushed two fighting students apart and did not use any guidance to the behavior.

During inspection, hand sanitizer, Lysol, and disinfecting wipes were observed in reach of children.

During inspection a caregiver left a group of children unsupervised for a few moments.

During inspection a room was being used for care that is not on the permit.

During an inspection broken toys and riding toys were observed on the playground. These toys were removed during the inspection. A broken window was observed on the building that is accessible from the toddler playground.

Four records were reviewed and all four need a copy of a photo ID. The director needs a copy of current driver's license.

Four records were reviewed and #3 and #4 are missing an affidavit of employment.

During inspection four records were reviewed: #2, #3, and #4 do not have current CPR/1st aid training.

Four records were reviewed during inspection: #2 Address and phone for doctor; healthcare provider statement #4 needs an updated shot record.

During inspection it was found that all cubbies or hooks need to be labeled.

Four staff files were reviewed. #2, #3, #4 need 8 hours of preservice training and CPR/1st aid training. All need Abuse/Neglect Training #1 and any other employees who work in the infant room need SIDS/Abusive Head Trauma/Brain Development Training.

Diagrams showing all the required components of the evacuation plan was not available.

The Emergency Preparedness Plan is incomplete and lacking information.

The air conditioning unit on the playground must be inaccessible to children. The grass needs to be maintained to prevent unwanted animals from hiding in tall grass and weeds. Mow, weed eat, and pull up plants that are poisonous. Trim Tree if desired to help with shade. Remove Wiring in large room.

Individual storage for children is not available.

The Parent Handbook does not have information about required immunizations.

The infant room has a wall that needs border put up to cover up torn walls at the base of the floor where paint can be pulled off and put into an infants mouth. There is also a place under the window area that needs repaired or covered. The entrance to the infant room has carpet that is frayed and needs to be covered. The large room has a wall that h as holes and peeling paint on the bottom that needs repairing or covering. The girls restroom has plaster on the wall by the left sink that needs repaired or covered.

Procedures for how parents can participate in activities at the center is not provided in the handbook.
