A review of the Employees Who Left Employment in 2025 data from January 20, 2026, determined the center had not reported the number of employees who left employment in 2025 by January 15, 2026, as required.

While the children were waiting for lunch to be served the TV was on; however there were 3 children under the age of 2 years old who were also watching. (NOTE: The TV was turned off)

There is no verificatio that the director is working at this operation. Director's background check has been run at another location.

The diaper changing table has a blue disposible mat which needs to be changed. The mat is dirty and has not been changed.

There was 1 caregiver in charge of 16 children, ranging from 12 months to 5/6 years old.

There was a restroom that was being used as storage since the sink had been removed. The playground was not maintained as there was some trash and the playground equipement was showing signs of dirt to show that they had not been cleaned.

The permit holder has not ensured compliance as the director has not been at the operation and does not have on-site responsibility of the child care center.

There was a child who is now 14 years old inside the daycare without the needed background check.

There were 2 classrooms in use for naptime, however the caaregiver was standing in between 2 doorways supervising both groups. (NOTE: Corrected during inspection as a 3 caregiver arrived to supervise one of the classrooms.)

The fire extinguishers in the building have not been serviced. They were last serviced in January 2021.
