The provider did not certify the Children's Product Certification form annually.

The fire extinguisher was not mounted.

The use zones in the front and the back of the swings were not at least 12 feet.

The use zone for the slide was not at least 6 feet.

One household member did not have proof of orientation.

There were no feeding instructions for a child not yet ready for table food.

Four of the five children files reviewed were lacking health statements.

It was found that one infant,not old enough for table food did not have feeding instructions. An infant feeding instruction form was emailed to the operation.

The provider has not certified in writing there are no unsafe or recalled items. This was completed during the inspection and there were no unsafe items.

The provider did not have current training in SIDS, Shaken Baby, Brain Development, Child Abuse and neglect, Emergency Preparedness, Preventing and controlling the spread of communicable disease, Administering medication, Preventing and responding to emergencies due to food or an allergic reaction, Building and physical premises safety, Handling, storing and disposing of hazardous waste material.
