A caregiver did not have a current CPR training.

The main mobile phone from the child care center has not remained in the building during operating hours.

Based on the information gathered during the investigation, the operation was cited for AP background check results for allowing an individual to have direct contact and transport a child from the school to the childcare facility without undergoing a background verification process. It was reported this individual was also present at the operation.

The permit holder did not adhere to the regulations outlined in the Texas Administrative Code concerning background checks.

There was one caregiver caring for 7 infants. The specified age for the group was 12-17 months which allows 5 infants to 1 caregiver. Note The ratios were left in compliance as an additional caregiver was put in the classroom.

There was bucket of water (left over mopping water) outside a classroom. There was a container with water (4 qt) and rag inside on top of a shelf outside the classroom. Note: The bucket and water container were emptied.

A child was unsupervised in a classroom which is near the exit door. The child was hanging from a half-wall in the classroom.

Based on the information gathered during the investigation, the operation was cited for AP background check results for allowing a person to be present at the operation without a background check.

One caregiver did not have the following required annual training topic for training period, 11/18/21-11/18/22: Prevention, recognition, and reporting of child maltreatment

Two children records reviewed did not have the address for the emergency contact on file. Corrected at Inspection: Records were updated.

The outdoor play area was observed with dog feces. There were Christmas lights on a playhouse which pose a hazard for the children. There was a broken wooden fence panel in the outdoor play area.

A caregiver did not have the following required training topics for training period, 5/27/2021-5/27/22: (1)Recognizing and preventing shaken baby syndrome and abusive head trauma; (2)Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS); and (3)Understanding early childhood brain development.

The outlets in the toddler classroom were not covered. Corrected at Inspection: The outlets were covered.

The infant caregivers ignored a crying infant for a high number of minutes.

Five infants were not strapped on their seats on the high elevated table. Note: The infants were strapped during the inspection.

A new caregiver did not have a First Aid Training and was unsupervised with a group of children.

Two caregivers did not have a training records available for review.

The transportation vehicle being used does not have a child safety alarm.

The activity plan for the toddler and 4-year-old classroom was not current. Corrected at Inspection: The current activity plan was posted.

A caregiver hired on 6/17/22 did not have their education documentation available for review. A caregiver hired on 5/13/21 did not have the affidavit for review. A caregiver did not have the following required annual training topic available for review for the training period, 5/27/21-5/27/22: Child Maltreatment.

The Food Allergy Emergency Plan for a child with a diagnosed food allergy was not posted at the operation. Corrected at Inspection: The food allergy plan was posted in every classroom in which the child is present.

The outdoor play area was observed with a broken playhouse with sharp edges, 2 loose screws on a playhouse accessible for children, and there was a small hill of red ants.

Five children's records did not have the required Admission Information regarding the special needs section.

Director did not make reasonable efforts to abide by the suspension/expulsion operational policy to prevent the expulsion of a child from the operation based solely on behavioral concerns.

The outdoor play area was observed with red ants. There were two little screws accessible to the children which pose a chocking hazard during outdoor play. Note: The screws were removed. There was deep hole on the ground which pose a tripping hazard during outdoor play.

The infant daily report was missing the section, Summary of Activities. Corrected at Inspection: Infant Daily report was updated.

A child with a diagnosed food allergy did not have a Food Allergy Emergency Plan.
