During the inspection it was observed a track rides equipment did not have at least seven feet from the perimeter of the equipment when not in use.

Three out of eight employees did not have notarized affidavits in files.

The playground maintenance checklists was not available at the inspection time.

One child was missing the vaccinations records in the file.

Four employees were missing orientation trainings in their files.

A review of the Employees Who Left Employment in 2024 data revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

One caregiver is missing 8 hours of annual training.

One caregiver employed over 90 days is missing 14 hours of preservice training.

2 caregivers are missing the following topics of required training: 1) Prevention, recognition, and reporting of child maltreatment 2) Administering medication 3) Emergency Preparedness 4) Preventing and responding to emergencies due to food or an allergic reaction

One caregiver present did not have a background check.

The fire inspection expired August 3, 2021.

During nap time there was a 12 month old infant and two 17 month old infants napping in the same room as a 4 year old. The infants were moved out of the room immediately.
