Records were reviewed the children's Product Certification had not been reviewed since 10/7/2019. NOTE: This was corrected at inspection when caregiver reviewed the children's products through the website and signed and dated the new form.

An infant in a crib was observed watching a cartoon show playing on the television in the infant room. Note: This was addressed with the caregivers and the television was turned off.

The loose-fill material (rock pebbles) were not maintained at the proper depth of 6 inches for two slides in the outdoor playground. The loose-fill material measured 2 1/2 inches in depth.

2 of 6 children's immunization records reviewed were not current.

A caregiver was missing annual training for training year 10/07/2020 - 10/06/2021. The training missing was on the following topics: (1) Recognizing and preventing shaken baby syndrome and abusive head trauma; (2) Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS); and (3) Understanding early childhood brain development.

The director was missing annual traininig in the following topics: Prevention, recognition, and reporting of child maltreatment. (1) Emergency preparedness; (2) Preventing and controlling the spread of communicable diseases, including immunizations; (3) Administering medication, if applicable, including compliance with 746.3803 of this chapter (relating to What authorization must I obtain before administering a medication to a child in my care?); (4) Preventing and responding to emergencies due to food or an allergic reaction; (5) Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and (6) Handling, storing, and disposing of hazardous materials including compliance with 746.3425 of this chapter (relating to Must caregivers wear gloves when handling blood or bodily fluids containing blood?).

Two caregivers Pediatric CPR training was completed online and did not adhere to the guidelines for CPR for a layperson established by the American Heart Association, and consist of a curriculum that incudes use of a CPR manikin and both written and hands-on skill-based instruction, practice, and testing.

The annual fire inspection report available for review was not current. The last fire inspection was conducted on 1/07/2020.
