The nursery classroom was over ratio by 2 infants. This was corrected at inspection once the infants were moved to another classroom.

2 infants were observed sitting in the bucket seats without safety straps fastened.

There were 4 caregivers lacking between 2 to 16 hours of pre-service training.

Based on the evidence obtained, there is sufficient evidence of caregivers on their personal cell phones and not attending to the children in care.

An infant was observed with a bottle inside of the crib while sleeping. This was corrected at inspection when the bottle was removed from the crib. An infant was observed walking around with a bottle.

The toddler playground and pre-k playground contained jungle gym play equipment utilized by the children.

It was observed a caregiver talking on the cell phone in the classroom while children were in care. It was observed that a caregiver walked out of the classroom leaving children unattended. It was observed that a caregiver was not actively supervising children during naptime, the caregiver's back was towards the children while sitting in a chair for more than 2 minutes.

It was observed that a bottle was in a crib while a child was sleeping.

Operation staff with conditions on the background check was left alone to provide care for children. An administrative penalty will be recommended as a result of this citation, per HRC ?42.078.

During the course of a DFPS investigation it was found a caregiver responsible for a group of children in care was using their cell phone to take selfies.

During an investigation it was found the permit holder is not familiar with the minimum standards.

During the inspection a caregiver was observed stepping outside of the classroom leaving the group of 9 children ages 1 1/2 to 6 unsupervised.

While doing a walkthrough there were holes in the ground in the outdoor play area.
