The first-aid kits in two transportation vehicles were incomplete. The items missing were tweezers, and Waterproof, disposable gloves. NOTE: This was corrected during the inspection as the missing items were obtained and placed in the first-aid kits.

A food allergy emergency plan was not available for review for a child with a known food allergy that has been diagnosed by a health-care professional.

A play yard being used by an infant did not have a fitted sheet on the mattress.

The emergency preparedness plan reviewed did not include specific procedures for evacuating and relocating children who are under 24 months of age, who have limited mobility, or who otherwise may need assistance in an emergency, such as children who have mental, visual, or hearing impairments.

A list of a child's food allergies was not posted in the room where the child may spend time and kitchen. NOTE: This was corrected during the inspection as a list of a child's food allergies was created and posted in the required room/areas of the operation.

Based on the information obtained during the course of the investigation, a caregiver pulled a toy out of a child's hands without providing positive discipline / guidance.

One out of seven children's records reviewed is missing immunization records.

Menu posted did not match the food items served during breakfast and lunch.

Potty chair was not sanitized after a child used it. Note: This was corrected at inspection when this was brought to provider's attention and the potty was sanitized.

A child did not have an individual / designated napping mat. Note: This was corrected at inspection when a mat was designated and labeled for the child.

Operation has three structures in the playground: structure with slide, multi-person spring rider and stationary car. Use zones for structures overlap and do not meet the required 6ft for each of the structures. Note: This was corrected at inspection when provider removed the structure with slide and stationary car.

Emergency plan is incomplete and missing specific steps for lock down . severe weather and evacuation procedures.

Operation did not have a current fire inspection. Last fire inspection on file was on 6/04/20.

The person in charge of transportation did not have a current transportation training.

The operation does not have a current health inspection. The last health inspection was conducted on 12/27/19.

An new employee hired did not have an orientation.

The infant daily report is not being shared with the parent.

Lock down drills were not conducted for the calendar year, 2020.
