The operational policies reviewed did not address the Parent Rights.

The most recent inspection form was not posted. The inspection form posted had a date of 9/27/2022.

Monthly fire drills are not being practiced. The last time a fire drill was practiced was on June 2025.

The outdoor area designated for the children was observed with unkempt with a wheelbarrow and BBQ pit.

A 4 month old infant was asleep in a car seat, restrictive device. Note: The infant was placed in a crib.

Personnel record for primary caregiver was not available for review. Dog vaccinations were not available for review.

The diaper changing mat was observed with brown and yellow stains. Corrected at Inspection: The diaper changing mat was cleaned during the inspection and the stains were removed.

There was a dog in the restroom used by the children. There was strong urine smell from the blanket used by the dog. The closet doors in the restroom were broken and can potentially fall on top of the children. Note: The dog was taken out of the restroom. The doors were removed from the restroom.

There was a bucket of water in the room used by the children. Note: The bucket of water was emptied. There were two knives in the kitchen counter accessible to the children in care. Note: The knives were removed and stored away.

The outdoor play area was observed clean. There were cleaning products under the kitchen sink and the cabinet door was broken. It poses a risk to the children during meal preparation.

The following policy was not included in the operational policies: (8) Promotion of indoor and outdoor physical activity that is consistent with Subchapter F of this chapter (relating to Developmental Activities and Activity Plan). Your policies must include: (A) The duration of physical activity at your home, both indoor and outdoor; (B) The recommended clothing and footwear that will allow a child to participate freely and safely in physical activities; and (C) A plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor time.

The CPR/First Aid Training Certificate did not include the required Pediatric CPR & First Aid training completed.

Primary caregiver did not have the following required annual training topics for the training period, 4/2021-4/2022: -Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS) -Understanding early childhood brain development. -Preventing and controlling the spread of communicable diseases, including immunizations -Administering medication -Preventing and responding to emergencies due to food or an allergic reaction; -Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic -Handling, storing, and disposing of hazardous materials

The outdoor play area was observed with construction material which pose a safety hazard for the children during outdoor play. There were cleaning products under the kitchen sink which does not have a cabinet door.

There was no admission information for a child in care.

There was no current CPSC Annual Certification.

Primary caregiver did not have the required annual training hours for the training period 4/04/20 to 04/04/21.
