The operation had a see-saw that rocks on the outdoor playground that did not have 6 feet of use zone space. This was corrected at inspection when the director moved the seesaw to have 6 feet of use zone space on all sides.

One child with diagnosed food allergies did not have a Food Allergy Emergency plan signed by the parent and health care professional.

Four children's admission files were missing the name, address, and telephone number of the child s physician and an emergency-care facility.

One classrooms had garbage accessible to children as there was a trash can that contained garbage and did not have a lid on it. The caregiver put the lid on the trash can during inspection.

One medication for a child with an allergy was not in the original container labeled with the child s full name and the date brought to the operation.

The loose-fill was not at the required depth per Minimum Standards underneath the exit of the slide. This was corrected at inspection when the staff added more loose-fill underneath the slide exit.

One caregiver hired after 9/1/23 did not have the Pre-Employment Affidavit form 2912. The director had the caregiver complete the affidavit during inspection.

Two caregivers did not have annual Handling, storing, and disposing of hazardous materials training.

The operation has 13 chickens and 12 roosters, 1 rooster was not in an enclosed coop/cage and was walking around the coop where children collect eggs from daily. The children also collect eggs daily from the chicken coop.
