The operation did not have a current children's product form on file certifying the recall notices were reviewed.

Based on information obtained during an investigation, it was determined that two caregivers left a child unsupervised on the playground for approximately 20 minutes when transitioning the children back inside.

The operation did not have a copy of their current liability insurance available for review at the time of the inspection.

One child's record lacked the name, address, and telephone number of the child's physician or an emergency-care facility

The operation's current fire inspection report was not available for review at the time of inspection.

Two caregivers were missing trainings in the following topics: *Administering Medication *Preventing and controlling the spread of communicable diseases *Preventing and responding to emergencies due to food or an allergic reaction One of the caregivers was also missing prevention, recognition, and reporting of child maltreatment

Documentation of the fire/CO drills, & extinguisher checks, was not available at the time of inspection.
