The director did not have the following trainings: *handling, storing, & disposing of hazardous materials *preventing and responding to emergencies due to food or an allergic reactions

An infant was left at the operation. The operation's license is not permitted to care for infants.

Based on information obtained during the investigation, it was determined that a caregiver inappropriately disciplined a child in care when the caregiver hit and/or pushed the child.

Two children did not have a health statement on file.

The operation's written emergency preparedness plan did not include all the required information.

Four caregivers did not have the following training: *Administering Medication

Based on information obtained during an investigation, this standard was found to be in violation. The operation failed to notify the parent of a child immediately after the child was involved in a situation that placed the child at risk. The parent was not notified until the next day.

Based on information obtained during an investigation, this standard was found to be in violation. The caregivers did not maintain knowledge of the children in their group and were not aware of which children were in the restroom and which children were in the classroom to ensure all children were accounted for. The children were able to freely go back and forth without staff accounting for their whereabouts.

Based on information obtained during an investigation, this standard was found to be in violation. The caregivers failed to properly supervise a child while in care and as result, the child was able to walk out of the classroom and exit the building through the front door without knowledge of the caregivers. The caregiver's responsible for the child were engaged in a conversation that distracted them from their caregiver responsibilities. The child was found outside in the front parking lot area by another caregiver.

Based on information obtained during an investigation, this standard was found to be in violation. Staff did not interact positively with children in care as they spoke harshly to children that were drinking water and/or taking too long to wash their hands.

Based on information obtained during an investigation, this standard was found to be in violation. A child was able to walk out of the classroom, out the building, and into the parking lot unnoticed. The child was unsupervised for approximately 2 minutes 25 seconds before being found in the parking lot area by another caregiver. The 3 caregivers were found Reason to Believe for Neglectful Supervision.
