A fire drill had not been conducted in July. This was corrected at inspection when was done and the children were able to exit within 3 minutes.

Primary and substitute caregivers were missing the annual child abuse training.

Primary caregiver did not have proof of current transportation training.

Caregiver took two children to the store without getting signed permission from parents.

Children's records were not being properly maintained.

Primary caregiver's licensed capacity is eight children and during inspection there were nine children in care. This was corrected at inspection when the child's mother was called and the child was picked up.

Water was not served during snack.

A toddler was sleeping on a cot with a bottle in his mouth. This was corrected at inspection when the bottle was removed.

All sleeping mats were not labeled with children's names. This was corrected at inspection when they were all labeled.

Primary caregiver allowed an unqualified substitute caregiver, help care for children.

There was an activity gym outside that had a large area of bird feces on it and children were observed playing on it. A photo was taken.

A mosquito and fly killer spray was placed on a shelf next to the day care area and was not stored in a cool dry place according to the label directions. A photo was taken. This was corrected at inspection when it was removed and stored away in a cabinet.

Recommended clothing and footwear for physical activity was not included in operational policies.

A plan to ensure physical activity occurs when weather conditions prohibit or limit outdoor time was not included in operational policies.

An activity gym outside was cracked and had an exposed sharp edge. This was corrected at inspection when crack was covered with tape. Photos were taken.

First-aid kit did not have cotton balls.

Health check procedures were not included in operational policies.

Provider was found caring for seven children from ages six months to four years of age during the hours of 6:00AM to 6:00PM, Monday to Friday. Children included 4 four year old children, 1 three year old child, 1 twelve month old infant, and one six month old infant. Care began three years ago and compensation was provided as Provider was paid $80 per child per week; siblings were $70 per child.
