Five out of twelve children's records were missing the emergency contact address. The Director corrected this at the time of the inspection by calling and getting the addresses.

One out of twelve children's records did not have a complete immunization record. The Director is going to correct this by getting the complete immunization record.

The diaper changing mat in the infant room had some areas on the mat that were porous which could cause cross contamination. The Director corrected this at the time of the inspection by replacing the diaper changing mat.

There were two trash cans with garbage that was accesible to children that did not have lids. The Director corrected this at the time of the inspection by getting lids for the trash cans.

One out of twelve children's records reviewed was missing a health statement. The Director is going to correct this by getting the health statement.

It was viewed during the observation of video footage caregivers playing on cell phones and ipads when supervising children.

It was seen during the viewing of video footage a caregiver using prohibited punishment.

It was seen during the video and the caregiver admitted to two other staff that she pulled and child by the back of his shirt and he fell to the ground. It was also seen in the video that she was harsh in which manner she would move him to sit on a bench.
