Observed diaper change done by caregiver for the children. The teacher did not wash her hands or childs hands after diapering and in between each child.

Caregiver stated the director was the one in charge but was not present.

One out of six children's records reviewed is missing immunization records.

Six staff files were reviewed: two files are missing translation / equivalence of foreign diplomas, one file is missing a valid high school diploma and one file is missing a notarized affidavit.

Children in the prek room were watching PJ masks.

Operational policies were reviewed and are missing the following items: -Vision and Hearing Screening Policy -Sunscreen and Insect Repellent Policy -Employee's Vaccines Policy

Two out of four staff training reviewed have certificates for CPR training completed online without hands-on skill-based instruction, practice, and testing.

The following trainings were not available for review for Director and one caregiver in charge of toddler children 24 months and younger. (1) Recognizing and preventing shaken baby syndrome and abusive head trauma. (2) Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS) (3) Understanding early childhood brain development.

The children's products certification was not completed timely. Last certification was completed on 08/5/2019.

Five bottles of cleaning supplies were kept accessible to children inside a classroom's restroom.
