A review of the Employees Who Left Employment in 2024, revealed the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

The operation do not have complete their annual children product certification for the year of 2023.

A total of seven operational staff files were review. Four of the seven operational staff do not have a valid CPR certification for review.

The basketball hoop was observed to be damage inside the school age playground.

Ratios were not maintained during the follow up inspection. I observed 2 children under 17 months old in the infant room and 10 additional children between 2 and 3 years old in a mixed age group. Note: It was discussed with the provider that the maximum number of children allowed is 4 children between 18 months-4 years old and 4 children who are 5 years old and up.

During the follow up inspection, the director did not know the number of children present in a classroom. Note: Caregivers must know the number of children they are responsible for at all times.

I observed two bottles of Lysol spray left on shelves in the classroom which were in reach of children. Note: This was corrected during the inspection.

During the follow up inspection, supervision was not maintained in a classroom with children. Note: A caregiver went back into the classroom during the inspection. Discussed that because a caregiver cannot see children from going to open the front door, another caregiver must step in to ensure supervision of children is maintained at all times.

During the follow up there were 3 staff records that still do not contain the following training topics: Handling, Storing and Disposing of Hazardous Materials Responding to Food Allergies Emergency Preparedness Understanding Building and Physical Premises Safety Preventing the Spread of Communicable Diseases

An 11 month old was observed in a crib with a blanket and a pacifier that included an attachment during the inspection. Note: The provider removed the blanket and the pacifier during the inspection.

The service light is on in the van to the child safety alarm.

The alcohol wipes in the first aid kit were expired.

1 out of 6 staff files evaluated did not contain completion of orientation.

The diaper changing mat in the toddler room was torn and cannot be cleaned properly.

A child was observed sleeping on the wrong nap mat.

The first aid kit in the van is missing tweezers.

Transportation training for the director and one caregiver who transports children was not completed within the evaluation period. An evaluation period between January 13, 2021 to January 13, 2022 was reviewed for the director and February 13, 2021 to February 13, 2022 for the caregiver.

A fire drill has not been conducte since February of 2022.

3 out of 8 children's files evaluated were missing current immunziations.

The smoke detector and carbon monoxide detectors have not been tested within the last month.

During the inspection the lighting was too dark in the pre-kindergaten classroom during naptime. Upon entering the room children could not be seen. Note: The provider turned a light on during the inspection.

An electrical outlet was left uncovered during the inspection. Note: The provider corrected this during the inspection.

Lysol spray and wipes were observed in reach of children on a shelf in two classrooms during the inspection. Note: The provider removed the cleaning supplies during the inspection.

One staff was missing orientation information on file out of four staff files checked.

There was no acknowledgement that one staff received the operational policies.

The director has 22 out of 30 clock hours of annual training from 8/2020 to 8/2021. Director lack 8 hours. Another caregiver had 18 out of 24 clock hours of annual training from 9/2020 to 9/2021 missing 6 hours.

Date of admission was missing on one out of four files checked. This was corrected during inspection.

Notarized affidavit was missing on one out of four staff files checked.
