One out of the four children records did not have their immunization records on file at the time of inspection.

Two out of the four staff records reviewed did no have the following trainings: (1) Emergency preparedness, (2) Preventing and controlling the spread of communicable diseases, including immunizations, (3) Administering medication, if applicable. (4) Preventing and responding to emergencies due to food or an allergic reaction, (5) Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and (6) Handling, storing, and disposing of hazardous materials.

One of the children's files evaluated did not have a signed health statement on file.

Documentation of completed gas leak test was not available for review.

After naptime, between toileting and transitioning for snack time, supervision was not observed to be appropriate at all times. A couple of children were observed climbing on furniture and running in and out of the classroom.

Two caregivers present did not have a notarized employment affidavits on file.

The operation did not have an updated maintenance checklist ready for review during the inspection.

The outdoor loose fill did not have the adequate depth for the play structures.

A garbage bag filled with used tissue paper and empty water was hanging in the outdoor playground accessible to children. This was corrected at inspection when director removed the trash bag from the active play area.

The infant caregiver was observed supervising one 4 month old infant in the infant room as well as 6 toddlers, all age 2 years old in the toddler room while sitting in between the two classrooms. NOTE: This is a second citation and was corrected at inspection by additional caregiver returning to the operation and stepping back into the toddler room bringing both the infant and toddler room back into child/caregiver ratio.

The director and one caregivers first aid/CPR certification expired on 8/31/21. One caregiver has not yet completed her first aid/cpr certification and was observed responsible for a group of children without another approved caregiver. Caregivers start date was 9/30/21.

The director was lacking 30 of the required 30 annual training hours towards the 01/2020-01/2021 training year.

One child's admission record was observed without a current health statement signed by a physician and was enrolled on 9/25/2019.

One caregiver had completed 7 hours towards the 5/2020-5/2021 annual training year and was lacking 17 hours.

The infant caregiver was observed out of the infant room and near the doorway of the toddler/preschool room, leaving a 4 month old infant unsupervised for an unknown amount of time. NOTE: This was corrected at inspection by caregiver stepping back into the infant room.
