The first aid kit had expired supplies inside. This was corrected during the inspection, when the supplies were removed.

Two wall coverings did not have wall protectors. This was corrected during the inspection when the electrical outlets were covered.

While review records it was found that 3/7 children files reviewed did not have their current immunizations records on file.

During the inspection it was found that the operation did not have a current posted menu available for review.

During the monitoring inspection it was found that there were 3 outlets that were not covered and there was an outdoor plastic playhouse that had cracked, leaving sharp edges. This was corrected at inspection.

While reviewing files at the monitoring inspection it was found that 2 out of the 9 files reviewed did not have signed admission forms.

During the inspection there was a person present who was observing a classroom for potential hire at the operation with direct access to the children before their FBI fingerprint results were in. The person was not being counted in ratio.

Observed a trashcan in the bathroom with dirty pull ups in the trash can without a lid, which made the diapers accessable to the children in the class. This was corrected when a lid was placed on the trashcan.

Observed a bathroom that had blinds on the floor,a pile of clothes on a shelf and a empty mop and mop bucket. The director removed the blinds, and mop bucket.
