Based on the video footage of the playground on 6.21.2024, a caregiver was observed pulling a child up by their left hand to standing position after the child attempted to sit themself on the ground.

Caregivers did not use good judgement when they did not notify the director or parent of a child's continued complaints of pain to their left arm.

The use zone of the swing overlapped with the use zone of the play tunnel on the playground.

Two of eight staff records reviewed were missing annual training for administering medications and responding to emergencies due to food or an allergic reaction.

A table was overlapping the use zone of a slide on the playground.

Monthly carbon monoxide detector checks were not documented when completed. Last carbon monoxide detector check was last documented on 6/1/23.

3 out of the 8 staff records reviewed were missing annual training in the required following topics: (1) Emergency preparedness, (2) Preventing and controlling the spread of communicable diseases, including immunizations, (3) Administering medication, if applicable,(4) Preventing and responding to emergencies due to food or an allergic reaction, (5) Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic & (6) Handling, storing, and disposing of hazardous materials

The emergency preparedness plan was missing specific procedures for evacuating and relocating children who are under 24 months of age, who have limited mobility, or who otherwise may need assistance in an emergency, such as children who have mental, visual, or hearing impairments.
