The last date the 2 fire extinguishers were serviced was in November 2024.

Upon arrival, two caregivers were on one side of a split room with 13 children and the youngest was 10 weeks old and the oldest was 4 years old which made them not be able to combine the infants due to the age of the youngest and the oldest in the room.

There was no orientation training available for review for one employee.

The last fire inspection was conducted on 12/17/24 which a current one has not been set up.

Upon arrival, a child was observed blocking the exit door while sleeping. This was corrected when the child was moved by the director.

Four children's records did not have the health statement information completed.

Five children were missing the emergency contact information. This was corrected when the director contacted parents for this information.

A caregiver was seen on live stream video tossing a blanket over a child during naptime twice when the child did not want to take a nap. Note: This caregiver was terminated.

Children were observed coming indoors from outside play and not washing their hands.

The employee was observed not washing hands after coming in from outdoor play.

The playground had overlapping play structures that did not extend a minimum of six feet in all directions. This was corrected when some of the equipment was moved out and moved 6 ft from the fence.

The playground had a few broken toys that were sharp to the touch and accessible to children. This was corrected when the toys were removed from the playground.
