At the time of the inspection 2 of the 10 children's records reviewed did not have up to date immunization records on file.

At the time of the visit the children's records were not maintained with updated information for all children reviewed.

Based on information obtained it was found that the building at the operation has not been maintained to be free from hazards. On the playground an area of the surfacing has been ripped open, with parts of the surfacing being raised and creating a tripping hazard. In the same area where the surfacing is ripped, there is a large dip in the ground that is hard to see unless you are on it. This creates a tripping hazard for the children. An area in the infant classroom has laminate tile that has raised and broken edges.

Based on information obtained the operation had requested repairs for areas of the building in playground more than 6 months ago and the repairs have not been made timely.

At the time of the visit 1 child's file indicated a food allergy but there was no food allergy plan on file.

1 of the 10 children's files reviewed did not have an immunization record for the child on file.

Based on information obtained through DFPS investigation it was found that children were not being changed frequently enough that skin irritation or diaper rash was caused.

At the time of inspection a caregiver was heard telling children that since no one took a nap they would not be getting chocolate. The caregiver was spoken to regarding the comment.

Based on information obtained during an investigation it was found that a child in care had been injured in July of 2022 and required medical treatment from a professional that the operation did not report to Childcare Regulation.

A caregiver showed poor judgement when sharing information of an incident occurring at the center with someone outside the center, instead of with the director. This type of action prevents necessary intervention when incidents occur in the center.

It was found that a caregiver used their personal device during the nap period to send messages to persons outside the center while counted in ratio as the caregiver, which reduces the caregivers ability to supervise adequately.

A caregiver used corporal punishment on one child by striking the child on their bottom in an effort to correct a behavior. The same caregiver used a threat of corporal punishment with an object to scare a second child into behaving correctly in the classroom.

At the time of my visit the child safety alarm on one of the operation vans was not working properly. The alarm did not sound when the van was turned off. A work order was put in during the inspection.

At the time of the visit 5 of the 6 children's cubbies in the infant classroom had used sippy cups or bottles with no label of the child's name they belonged to. This was corrected when the infant caregivers applied labels to each child's cup during the inspection.

At the time of the visit 2 of the 10 children's files reviewed had listed food allergies with no doctor's note indicating the treatment for the allergy.

This rule was evaluated with regard to an investigation and has been found to be in violation. The caregiver did not demontrate competency or self control when she grabbed a two year old child by the wrist to place her in time out.The caregiver pulled hard enough to injure the child's elbow.

The substitute caregiver did not have orientation to the center prior to being placed in charge of a class room. The director had the substitute complete the center's orientation to correct the deficiency.

The center did not have verification of a current gas inspection.

One bus driver did not have verification of current first aid training.

One bus driver did not have verification of current CPR training.
