Based on the information gathered the incident report was not shared with the parent.

In the infant room ages of the infants were 6, 6,6 10. and 10 months. The specified age group was 6 months. Ratio should have been 2:5 not 1:5.

A teacher left 3 one year olds unsupervised while she went to put up food.

The CPSC form has not been signed stating the CPSC website has been reviewed for recalled toys and furniture.

An electrical outlet was within reach of children. It was not covered. This was corrected during the inspection when an outlet cover was placed on it.

One infant feeding instructions was out of date. It was dated 06/04/25.

A caregiver did not share information with another teacher informing her the ages of the infants in care.

Infant bottles were not labeled with the infant's first name and the initial of their last name.

An 8 month old infant was laying on top of a blanket. The blanket was being used in place of a tight fitting sheet.

In the pre-k classroom the wall paint has been torn.

A teacher was asked how many children she had. She stated 8. It was verified by CCR there were 10 children in care.

A caregiver did not share information with another teacher informing her one child was leaving. This lead the teacher mis-counting the children she was responsible for.

The last inspection report was not posted.

Information was not shared with an incoming teacher with how many children were in care in the classroom that they were taking over.

An infant age 4 months was being fed in a bouncer while the teacher held the bottle sitting down in a chair.

There is no documentation available showing a grounds checklist has been filled out.

A teacher did not demonstrate good judgment by allowing a child under 5 to hold the bottle for a 4 month old infant while they were being fed.

Two 4 month old infants were watching TV.

Two staff caring for children have not received their eligibility letters. One teacher was in the infant room the other was in the pre-k classroom.

Two infants not on table food did not have written feeding instructions signed by the parent.

A teacher was asked how many children were in care and she could not answer how many children she was responsible for.

There were 5 infants in the classroom. The specified age group was 8 months. Ratio was 1:5 is should have been 1:4.

There is no first aid kit.

There is not a second exit on the playground.

There is no plan for safe route for children to exit the building while going to the playground.

CPSC form from HHSC has not been signed.
