Food allergy emergency plans are missing for three children diagnosed with food allergies.

Ten children's records were reviewed: one is missing immunization records, two are missing health statement and one is missing special care information.

In the prek room; two clorox bottles were stored underneath the sink cabinet and accessible to children. In the school age room; a cleaning product (Fabuloso) was stored underneath a sink and accessible to children. There was a decomposed pumpkin on top of a fridge and a expired gallon of milk inside the fridge. Note: This was corrected when cleaning products were moved and a lock was installed in the prek3 sink cabinet. Pumpkin and milk were disposed off.

Ten staff files were reviewed: two are missing preemployment affidavit, one is missing notarized affidavit, two are missing valid high school diploma and two are missing current first aid/cpr training.

Based on the information obtained during the investigation, it was determined the operation did not notify Child Care Regulation of an incident with a child walking out of the center unsupervised.

The use zone in front of the exit of a slide extended 4 feet 11 inches and not at least the required six feet.

The director and a caregiver who conducts transportation were not current in the required transportation safety training. The training was last completed on January 20,2023.

The first aid kit in one of two transportation vehicles was incomplete. Tweezers were missing from the first aid kit. Note: This was Corrected during the inspection as a pair of tweezers were obtained and placed in the first aid kit.

Soiled diapers were observed inside a trash can with no lid in a children's restroom. Note: This was COREECTED during the inspection as the trash can was replaced with another trash can that included a cover.

Several children in the pre-k room were not sleeping on their assigned floor mat.

An electrical outlet within children's reach did not have a childproof cover in the infant room. Note: This was CORRECTED during the inspection as a childproof cover was placed on the electrical outlet.

Toddler caregiver was observed to be using their cell phone while caring for four toddlers in the outdoor playground.

The emergency medical transport and treatment authorization forms were not available in the transportation vehicle binder for four children being transported. Note: This was CORRECTED during the inspection as the required forms were obtained and placed in the transportation vehicle binder.

1 out of 4 children's files reviewed was missing the child's health statement. 1 of 4 children's files reviewed was missing the child's current immunization record.

The Children's Products Certification form that certifies that the operation has reviewed each of the recall notices issued by the CPSC and that there are no unsafe products in the center was not current. The date documented on the form was 9/28/2020. Note: This was CORRECTED during the inspection as the director reviewed the United States Consumer Product Safety Commission (CPSC) recall list and updated the CPSC form.

A caregiver did not wash their hands after wiping a child's nose in the toddler room. Note: Hand washing was addressed during the inspection and the caregiver proceeded to wash their hands.

Two toddlers having lunch were observed sitting on high chairs that were missing the safety straps. Note: This was CORRECTED during the inspection as a toddler was moved to a high chair that included the safety straps. The second toddler was sat in a children's chair to finish their meal.

A child's soiled underwear, shorts, and shoes were observed on top of the hand washing sink in a children's restroom. Note: This was CORRECTED during the inspection as the soiled clothing was removed from the children's restroom and placed in a sealed plastic bag.

1 of 4 children's immunization records reviewed was not current. 2 of 4 children's records reviewed were missing the child's emergency contact address. Note: The children's emergency contact information was obtained and documented during the inspection.

The diaper changing mat in the toddler room was torn. Note: This was CORRECTED during the inspection as the torn diaper changing mat was replaced.

An electrical outlet in the infant room was observed with no safety cover. Two electrical outlets in the toddler room were observed with no safety covers, and three electrical outlets in the pre-k room were observed with no safety covers. This posed a safety hazard as the electrical outlets were within children's reach. Note: This was CORRECTED during the inspection as safety covers were placed on the electrical outlets.

The sheets on three play yards were not fitted and presented an entanglement hazard. Note: This was CORRECTED during the inspection.

Caregivers did not ensure continuity of care for children by sharing with incoming caregivers information about coverage during the caregiver's breaks. As a result 3 infants were left unsupervised in the infant room.

An 11 month old infant was observed asleep in a baby rocker.

Used toilet paper was overflowing from the trash can in a children s restroom. Used toilet paper was also observed on the restroom floor. Note: This was CORRECTED during the inspection when the trash was removed from the children's restroom.

There was no person in charge while the director was away from the center.

The director is not meeting director responsibilities as caregivers are leaving children unsupervised and not complying with minimum standards.

Three infants were left unsupervised in the infant room for an undetermined amount of time when the caregiver caring for the infants was observed in the afterschool room with another caregiver.

Both play yards in the infant room were not equipped with a tight fitting sheet.

Two infants were left unsupervised in the infant room for an undetermined amount of time when the caregiver caring for the infants was observed in the hallway walking towards the infant room. In addition 5 toddlers were left unsupervised in the toddler room when the caregiver in charge of the group was also observed in the hallway outside of the room.

There wasn't an additional adult in the building as required during naptime.

The play yard in the infant room was not equipped with a tight fitting sheet.

The lighting in the infant room and toddler room during naptime was not adequate to allow visual supervision of all children in the group at all times. Note: This was CORRECTED during inspection as the window curtains were moved to allow enough lighting into the rooms.

Ratios were not being met when there was more than an 18 month difference when older children and infants were combined. The oldest child was 10 years old and the youngest child was 16 months old.
