1 infant in care who is not yet eating table food did not have up to date infant feeding instructions.

At the time of inspection, the infant room in building one was observed to be out of ratio with 7 infants to 1 caregiver. This was corrected at inspection by having an additional caregiver go into the room to make 2 to 7 infants.

2 caregivers did not have CPR and First Aid training certificates.

In the Toddler 1 classroom there was a cleaning product spray bottle out on a low cart accessible to children. This was corrected at inspection by hanging the spray bottle on the back of the storage room door making it inaccessible to children.

There were 2 individuals present at the operation in the classrooms with children with a pending background check. This was corrected at inspection by having the 2 individuals with the pending background checks leave the operation.

One child lacked authorization for emergency medical care.

One child lacked health statement documetation.

One caregiver only completed 6 of the required 24 clock hours of training for the previous training year. One caregiver did not have any clock hours available for the previous training year. The caregivers will need to receive make up training that will not count towards the current training year.

The director's annual training hours for the previous training year did not include the following topics of training: *SIDS, shaken baby, & early brain development *Prevention, recognition, and reporting of child maltreatment *Emergency preparedness *Preventing and controlling the spread of communicable diseases *Administering medication *Preventing and responding to emergencies due to food or an allergic reaction *Understanding building and physical premises safety *Handling, storing, and disposing of hazardous materials

Six of the children's records evaluated did not include release information.

The center director and two caregivers did not have current cpr/first aid training on file.

Six of the children's record evaluated did not include emergency contact information.

Two infants did not have current feeding instructions on file.

Personnel files were missing information to include the following: *TB test *Proof of education *Notarized affidavit

The six children's records lacked the name, address, and telephone number of the child's physician or an emergency-care facility.

The operation allowed a person to be present at the operation after CBCU notified the operation that the person was "Ineligible" to be present. Note: The staff person left the operation during the inspection.

The operation did not have a current CPSC form showing they have reviewed recalls. Note: This was corrected during the inspection when the operation certified in writing by completing the CPSC form.

The operation did not have a current health inspection on file. The last inspection was completed on 9/23/20.
