Operation staff was observed to have expired adminstering medication training while administering medication to children.

Seven out of the nine currently enrolled infants had expired written feeding instructions.

During the investigation, it was confirmed that while two staff members were attending to another child, two children were able to squeeze out of the fence on the playground and exit to the parking lot.

A parent was not immediately notified that their child was able to squeeze out of the fence on the playground and exit to the parking lot.

During the investigation, it was learned that a written incident report was not completed following an incident where two children were able to squeeze out of the fence on the playground. This was corrected when the operation completed an incident report for each child and had the parents review and sign it.

During the investigation, it was learned that a gap in the fence allowed two children to squeeze through and exit to the parking lot. This was corrected prior to the inspection when the owner added an extra fence post in the gap.

The operation did not have a documented test for the previous month.

The fire extinguishers were not serviced by the one year tag.

Caregiver did not know the ages of the children in their care in the room.

One out of the six children records did not have their immunization records at the center for review at the time of inspection. Note: This was corrected when the parent brought the immunizations record to the center.

Four children did not have written feeding instructions at the time of inspection.

An infant was sleeping in the restricted device and caregiver did not know to immediately remove the infant from the device and place the infant in a crib as soon as possible. This was corrected when caregiver was informed of standard and she removed the infant from the restricted device and placed him in his crib.

There is video footage of caregiver stepping out the infant room leaving the infants unsupervised.
