Fire drill was not documented for the month of May 2025.

The sign in and out log was not maintained. Corrected at Inspection: The times and initials were placed for those who had not been sign in.

The fire safety documentation for emergency drills, fire extinguishers, and smoke detectors was not completed for the month of June 2024. Corrected at Inspection: The fire safety documentation was updated.

Training certificate was not available for review for an employee's record.

The transportation vehicle was observed with trash (plastic wrappers, popcorn, and chips) on the floor. Corrected at Inspection: The transportation vehicle was cleaned during the inspection.

A caregiver's foreign education was not translated. A caregiver's translated education document did not indicate the equivalency of education in the USA.

An infant was asleep in a play yard that did not have a sheet. Corrected at Inspection: The caregiver placed a sheet on the mattress of the play yard.

One cockroach was observed in the school age classroom. The outdoor play area was observed clean and free of any trash.

The school age children are not napping at the operation.

The outdoor play area was observed with trash (used napkins, a Styrofoam cup, and an old empty cigarette box.

A school age child was playing and chatting on a game app, Roblox, on their IPAD which was not part of the activity plan.

The toddler classroom was observed with dust and debris on two large vents.

The operation failed to notifying Child Care Regulations of a positive COVID-19 case in the operation. Note: Operation provided a written plan to notify CCR of communicable illness deemed notifiable by DSHS.

Loose fill surfacing (mulch) was not maintained at the proper depth for a play structure in the outdoor play area. There was no mark to indicate the depth of the mulch.

The exit door from the toddler room was stuck and unable to open. Corrected at Inspection: After several attempts a caregiver was able to open the door.

The door in the school age classroom had a broken hinge. The hand washing sink in the Preschool room was observed with a cockroach and cereal.

An infant was observed asleep in the crib with a pacifier attached to the infant's clothing.

A caregiver did not wash the infant's hands after diaper changing. Note: The infant's hands were washed during the inspection.

The exit door in the toddler room was locked and was unable to be open.

Two caregiver did not have a current background with eligibility results to be present in the operation.

A climbing equipment did not have the required depth of loose fill surfacing material. Corrected at Inspection: The equipment was removed.

The trashcan in the Preschool room was open and full which accessible to the children in the room. There was trash ( plastic cups and papers) observed in the playground area. There were also sharp tree branches at a child's height level which pose a hazard to the children during outdoor play. There was play equipment with an exposed bolt.

Director and person in charge of transportation did not have a current transportation training.

Several mats for the toddler and school age children were torn.
