Instead the caregiver called the parent to pick the child up for them to change.

During the course of the investigation it was discovered that a caregiver did not use good judgment when she did not check a child for injuries nor apply first aid to the child who had fallen off the slide and broke their wrist and thumb.

The paint in one classroom is chipping off of the wall. Nails/screws are protruding on the ramp on the playground.
