It was found that the caregiver did not complete name to face/count resulting in leaving a child outside playarea for 2 minutes.

While reviewing children's records, the inspector discovered a single-dose Benadryl tube without its original packaging.

I observed that the operational policies did not include information concerning: 1. TB Testing Requirements 2. Hearing and Vision Screening 3. Staff Vaccinations NOTE: This was corrected during the inspection.

I observed feeding instructions for an infant not yet on table food that had not been reviewed/updated by the parent in the last 30 days.

Caregivers were interviewed and procedures were evaluated. It was determined that proper child accounting procedures were not followed and a toddler was left on the playground during a transition time.

I did not observe documentation of Safe Infant Training for a caregiver who works with infants.

I observed that the center's operational policies didn't have information concerning the following: 1. Vaccines for Staff 2. Contact information for the local Licensing office and the Child Abuse Hotline. NOTE: This was corrected during the inspection.

I did not observe documentation of vision and hearing screening for a child who turned 4 before 9-1-20.

I did not observe documentation of an immunization record in 1 out of 4 children's records evaluated.
