Little Lobos Daycare

A child's pair of soiled shorts and underwear were observed over a wooden fence left to dry in the outdoor playground.

Based on the information obtained during the course of the investigation, it was determined that the operation did not discard a container of baby cereal after the expiration date. The expiration date on the container was July 31, 2025.

There was more than an 18 month difference when infants and toddler children were combined in the infant room. The youngest child in the group was 3 months old and the oldest child was 28 months old.

Five school aged children were observed on cell phones and tablet watching videos/shows as they were getting ready for nap time. The content being viewed by children was not age appropriate or related to the operations planned activities. A child was observed playing a game called Roblox, Two children were observed watching videos on You Tube and two other children were observed watching a show called Squid Games. Note: This was addressed with the staff and the electronics were removed and stored and away.

A caregiver's education documentation was not translated into English.

A caregiver was missing annual training on the following topics: (1)Prevention, recognition, and reporting of child maltreatment; (2) Emergency preparedness; (3) Preventing and controlling the spread of communicable diseases, including immunizations; (4 Administering medication; (5) Preventing and responding to emergencies due to food or an allergic reaction; (6) Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and (7) Handling, storing, and disposing of hazardous materials including compliance with ?746.3425 of this chapter (relating to Must caregivers wear gloves when handling blood or bodily fluids containing blood?).

The person in charge of transportation did not have a current driver license.

Three infants were not securely strapped in their chairs on the elevated multiple seater table.

There were 3 nails observed in the outdoor play area. Note: The nails were removed.

Three caregivers did not have the 24 hours of Pre-service trainings.

The wooden fence was repaired. The outdoor play area was observed with 5 cigarette butts and nail on the floor. It was reported the construction workers worked on the outdoor area the previous days. Note: The outdoor area was cleaned during the inspection.

The transportation vehicle/15 passenger does not have a child safety alarm.

There was no admission information including emergency medical transport and treatment authorization for five children in the transportation vehicle which are transported from school.

Training records were not available for some staff.

A section of the wooden fence was loose which poses a risk to the children during outdoor play.

Four infants were not securely strapped on their feeding chairs.

Medication records were not available for review.

The employee list was not current. Corrected at Inspection: The current employee list was posted.

The activity plan was not labeled for a specific group. The current infant activity plan was not posted. Corrected at Inspection: The current activity plans were posted.

The sign in/out log did not have the initials of the parent/caregiver. Correct: The sign in/out log was updated during the inspection.

Four out six childrens records were missing the address for the emergency contact.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

There was no Written Feeding Instructions for an infant in care.

The Infant Daily Report was missing the section for Summary of Activities.

The sign in and out log was not maintained. Corrected at Inspection: The times and initials were placed for those who had not been sign in.

The play yard used by an infant did not have a fitting sheet. -The play yard was not labeled with the infant's name. Corrected at Inspection: A fitted sheet was placed on the mattress and the play yard was labeled with the infant's name.

A Food Allergy Emergency Plan for a child was not signed by the Health Care Professional.

There was no gas leak inspection.
