A broken toy was observed in the playground area. NOTE: This was corrected at inspection the broken toy was removed from the playground and disposed of.

A review of the Employees Who Left Employment in 2025 data from January 20, 2026, determined the center had not reported the number of employees who left employment in 2025 by January 15, 2026, as required.

In the outdoor playground area there were 3 toy cars that were not kept in a sanitary manner. There was dirty water in the car, making it unsanitary for the children to use. NOTE: This was corrected at inspection when employee removed the cars from the outdoor area and threw them away.

During inspection 2 garbage bins in the classrooms were seen without the lid, making it accessible to the children. In the restroom a trash been was seen without a lid making it accessible to the children. NOTE: This was corrected at inspection when caregiver bought new garbage bins and removed the old ones.

Children's records were evaluated 1 out of the 4 records were missing the health care professional statement.

There was a water bucket filled with water at the operation's playground. Water bucket was on the grass and accessible to children. Note: This was corrected at inspection when provider dumped the water out of the bucket.

An employee ineligible to be present at the operation was present at the operation and participating in the care of children.

There is a stack of boxes and equipment in the prek classroom. Used water cups and broken boys were observed at the operation's playground. Note: used water cups were removed from playground during inspection.
