Little Harvard Educational Center Preschool and Day Care

The diaper changing mat used in the infant room had a polyester cover. The cover created the surface of the mat to become absorbent and hard to sanitize. Note: This was corrected during the inspection as the cover was removed from the diaper changing mat.

The firs-aid kit in both transportation vehicles were missing a thermometer, and scissors. The first-aid kit available in the center was missing tweezers.

Six staff files were reviewed and the following information was not available for review at the time of inspection: -Transportation training for Director -16 to 24 hour of pre-service training for 3 caregivers - 1 to 4 hours of annual trainings for 2 caregivers including trainings of safe sleep and abuse/neglect

Six children's files were reviewed. All files are missing the updated water activities information including the children's ability to swim details and special care information. Three out of six files reviewed are missing updated immunization records.

Infant-age children have not particpated in outdoor play.

Last fire drill and smoke/CO detectors and fire extinguisher checks were documented 12/14/2023. Per provider checks and fire drill were conducted on 1/25/2024 but were not documented. Note: This was corrected at inspection when provider documented the fire drill and checks from 01/2024.

Based on the information obtained during DFPS investigation it was determined a child was left unattended in a vehicle.

Based on the information obtained during DFPS investigation it was determined staff place children in highchairs as a form of discipline.

Based on the information obtained during DFPS investigation it was determined a child was involved in an incident that raised to the level of neglect under the care of the operation.

Some of the napping mats were touching each other leaving insufficient space for staff to walk in between the mats.

Training on SIDS / Shaken Baby was not available for review for two out of six staff training records evaluated during the inspection.

The role of a volunteer changed to an employee role two months ago. Operation has not submitted a renewal background check with the employee's current role.

Five children's records were reviewed. One files is missing emergency contact name, address and telephone number. A second file is missing immunization records and emergency food allergy plan and/or statement child no longer has a food allergy.

Upon arrival the TV in the toddler room was on and playing about indoor skydiving. There were children ages 18mo-2yo in the classroom. Note: This was corrected at inspection when TV was turned off.

Five staff files were reviewed: one staff members is missing 10 hours of annual training and a second staff member is missing 8 hours of annual trainings including trainings on the following topics: Management Hazardous materials Food allergies Administering medications Emergency Preparedness Building and Safety Premises Safety Communicable Diseases

Transportation binder taken during routes is missing list of the children being transported, authorization to be transported and receive medical attention during emergencies and children's emergency contact other than the parent.

Feeding instructions for a child no yet eating table food were last reviewed and signed by parent on 02/28/2022.

Last Fire Inspection was conducted on 12/7/2020.

During nap time in the after school group, the mats were arranged right next to each other with no sufficient walk and work space for caregivers between each mat.

An exit door was locked at the operation.

An infant was observed asleep in a bouncer and was covered with a blanket.

Emergency Preparedness Plan is missing the following sections: 2) Communication; (3) How your staff will evacuate and relocate with the essential documentation (4) How your staff will continue to care for the children until each child has been released; and (5) How you will reunify the children with their parents as the evacuation, relocation, or sheltering/lock-down is lifted.

Ratios were not being met when an infant was mixed in a group in which the oldest child was 4 years old.

Three children's records did not have their immunizations on file.
