Observeed two caregivers in a classroom with 7 children, one caregiver was asleep on the floor and the other was on their phone. This was corrected when the caregiver woke up and the other got off of their phone.

Observed two caregivers files without proof of education.

Observed training records, two caregivers were missing the following trainings, Caregiver 1- Child growth and Development, Discipline and Guidance, Age appropriate corriculumm, Teacher child interaction, Emergency preparedness, Communicable, Administering medication, Food allergies, physical building premisis safety, hazardous. Caregiver 2 Teacher interactions, emergency preparedness, communiccable disease, adminstering medication, food allergies, physical and building premises safety and hazardous materials.

Observed one caregiver that did not have a pre employment affidavit.

No one at the operation had a current first aid or CPR. Their training had expired in 9/2025.

Observed two caregivers on one playground with 2 children on their phones. The other 6 children were on a different playground. This was corrected when the phones were put away and the caregivers put all children in one playground.

Observed a television sitting on a shelf, I spoke to the caregiver about having the television mounted to the wall. The television was removed from the classroom.

Observed a caregvier rinsing cups in the handwashing sink. I spoke to the director regarding not rinsing the cups in the same sink that she washes hands in.

Observed a 5 month old infant sleeping in a restrictive device (bouncy seat). This was corrected when the caregiver placed the infant in the pack and play.

During the inspection, the diaper changing mat was observed with food particles on it.

During the follow up inspection, broken toys were observed on the playground area where the children had previously been playing.

A caregiver was observed alone in the room with 16 children with the specified age of 2 years old. The ratio for the specified age range of 2 years old is 11. A caregiver was observed alone in a room with 6 infants ages 0-11 months when the 2nd caregiver left the room to answer the door.

During the monitoring inspection, the play yards were observed and did not have a tight fitted sheet on them.

Two of the caregivers did not have orientation training.

While reviewing records, two of the five children files did not contain a health statement.

5 of the caregivers did not have active CPR or first aid training.

During the inspection, trash was observed on the playground area.

An infant was observed swaddled in a blanket while being held by a caregiver.

The infants were over ratio by one infant. There were five infants ages 4 months to 8 months old present and one caregiver.

During the inspection, the infants were observed in a room without a caregiver present.

During the follow up inspection, a 13 month old was combined in a room with children who were 4 years old.

During the follow up inspection, there was only one caregiver observed in the building as the children were napping.

During the monitoring inspection it was found that fire drills were not being conducted monthly.

One of the children's records reviewed did not contain immunization records.

During the monitoring inspection, an infant was observed in a room napping without an adult being physically present in the room.

During the investigation inspection there was only one caregiver in the building during nap time.

During the monitoring investigation there was an infant combined in ratio with older children during nap time.

During a walk-through of one of the outdoor playground areas, piles and bags of leaves were observed.

During a walk-through of one of the outdoor playgrounds, garbage was observed to be accessible to children. The garbage was removed when addressed, bringing the facility into compliance.

During a walk-through of one of the outdoor playground areas, yard equipment was observed that included a rake, a shovel, and broom. All items were removed from the playground area when addressed, bringing the facility into compliance.
