The loose-fill surfacing (shredded rubber) was not maintained at the proper depth of six inches. Areas around the play structure and slide exits had loose fill surfacing measuring 2-3 inches in depth.

The electronic smoke alarm system was not currently tested. The documentation available for review indicated the date of the last inspection was on 7/5/2023 and an inspection is due annually.

Eight children's files were reviewed. One file is missing health statement and a second file has a food allergy emergency plan that is missing the steps to take if the child has an allergic reaction.

Operation has a play-structure surrounded by 3-5 inches of tire mulch.

Children's Product Safety Certification Form was last signed on 7/14/2022. Note: Provider is signed up to receive email notification for all recalls and signed the form during inspection.

Operational Policies are missing procedures for conducting health checks.

Diaper changing mats in the infant and toddler rooms are torn.

Binder taken during transportation routes is missing emergency medical transport and treatment authorization forms for each child being transported. Note: This was corrected at inspection when provided added a copy of the 2nd page of the admission form to the transportation binder.

One out of six children's records reviewed is missing immunization records.

One out five staff files reviewed is missing a copy of high school diploma/GED or equivalent.
