During the investigation inspection the director left the operation and the person left in charge did not have access to all of the information needed by licensing. Person in charge was also counted in ratio.

During the investigation inspection it was found that the school age bathroom was not equipped with toilet paper and the caregiver did not have access to get any to replace it.

During the investigation inspection it was found that the operation's carpets were not kept clean and there were food crumbs and dirt visible on the carpets and floors. The bathroom in the school age room had toilet paper, dirt, and dried urine on the toilet, floor, and in an unused shower.

One caregiver was observed leaving a classroom of ten, 16 month-4 year old children unsupervised during nap while another caregiver stood between their two classrooms.

A caregvier was found unsing their phone for personal use while caring for children. This was corrected when the phone was put away.

There were no lesson plans posted for either classroom. The lesson plans were completed and online but not posted. This was corrected when the lesson plan was posted for each room.
