Five staff files were reviewed: time cards for one staff member, pre-employment affidavit for one staff member and ID for two staff members are missing.

Last fire drill and CO detector / fire extinguishers checks were conducted on 06/26/2025. Note: Provider completed and documented CO detector / Fire Extinguisher checks during inspection.

A caregiver's record reviewed was missing a signed and dated statement or certificate from the employee and the person providing the pre-service training stating the employee has received the pre-service training.

A caregiver's record reviewed was missing the caregiver's documentation showing how the caregiver meets the education qualifications.

Three cots and two floor mats used by children were not labeled with the child's name. Note: This was CORRECTED during the inspection as the caregiver labeled each cot and mat with the child's name.

A caregiver caring for a group of children did not have information available in the room showing each child's age. Note: This was CORRECTED during the inspection as a list including each child's name and DOB was created and made available for caregivers in the room.

The side loading door of the transportation vehicle did not open from the inside of the vehicle.

A caregiver's certificate of training in pediatric first aid and pediatric CPR was not available for review at the operation.

A caregiver did not wash an infant's hands after a diaper change. Note: This was addressed with the caregiver during the inspection and the caregiver proceeded to wash the infants hands.

The interior rear-view mirror in the transportation vehicle was broken off the windshield.

A caregiver's record reviewed was missing the following information: (1) Employment begin date (2))A statement signed and dated by the employee showing he has received a copy of the child-care center s: Operational policies; (3)A statement signed and dated by the employee showing he has received a copy of the child-care center s: Personnel policies; and (4) A statement signed and dated by the employee verifying the date the employee attended training during orientation.

The diaper changing mat in the infant room was observed with several tearing. This made the diaper changing mat absorbent and hard to clean.

Loose-fill surfacing materials (shredded wood products) were installed over asphalt. The loose-fill surfacing measured 1 inch in depth.

The surfacing material was not installed in the use zones for a play structure in the outdoor playground.
