There is a Lil Tykes type of structure that has a cracked in the middle section which poses a pinching hazards.

Out of children's files reviewed, 1 was missing the needed immunizations.

There is an old indoor AC blower in the corner of the playground accessible to the children. There was an old flower pot that was rusted at the bottom and had fallen over (this was removed from the playground.)

There were a total of 10 toddlers with 1 caregivver. The specified age was 22 months which caused the classroom to be out of ratio by 1. (NOTE: Corrected during inspection as the oldest child was moved into the next classroom.)

Staff files reviewed shows that they are missing training in child maltreatment.

There are 2 caregivers who rotate being caring for infants who do not have current training in SIDS.

A review of the Employees Who Left Employment in 2024 data from January 16, 2025, revealed the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

There was a napping mat in the afterschool room that had tears on top. It does not allow for proper sanitizing. (NOTE: corrected during inspection by throwing it away)

During a DFPS investigation, it was found that a child was hit on the head by a caregiver.

During a DFPS investigation, it was found that infants had been left unsupervised while caregiver was outside with a group of school-age children.

During a DFPS investigation is was found that the children in the 3 year old classroom were being yelled at.

Based on the DFPS investigation it was determined that the none of the staff understood who was in charge of the center while the director was out of the building.

Based on the information gained during the investigation, the caregiver's actions do rise to the level of abuse as defined by the Texas Family Code since the child was observed with marks and bruises due to inappropriate restraint.

During the course of the DFPS investigation, it was determined that the infant caregiver was in charge of infants and afterschool children with one of the youngest school age children being at least 6 years old with the infants at least 17 months of age.

The employee list was validated while inspector was present. (NOTE: Corrected during inspection.)

There were 2 areas on the playground fence that needs to have boards replaced. (Pictures on file)

The background checks had not been validated since May 23,2023.

Immunizations were not available for a child that is enrolled and attends day care.

Observed two caregivers present and in charge of a group of children in the operation without having had a background check /FBI. Observed a third employee present in the building without having a background check/FBI.

Admission information was not available for a child that is enrolled and attending day care.

Permit holder needs to ensure that facility complies with child/caregiver ratios and must allow parents access inside facility to drop off and pick up their children during business hours as well as comply with Minimum Standards and all applicable Laws and Rules.

Observed that facility does not allow parents to have access inside the building to drop off children.

Observed a caregiver supervising 6 infants.

Observed chlidren having breakfast and did not wash their hands.

Observed a caregiver supervising 9 infants. Child/caregiver ratios for infants 12 months to 17 months is 5:1.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

A caregiver was heard using an inappropriate tone with children in care.

Two caregivers do not have TB results available for review.

A caregiver does not have CPR training available for review.

Two children files are missing immunization records.

Director does not have annual training for cycle 8/24/20 to 8/23/21.

A caregiver's files was not available at the time of the inspection.

A caregiver does not have annual training hours for cycle 11/26/20 to 11/25/21. A second caregiver does not have annual training hours for cycle 9/12/20 to 9/11/21.

A caregiver does not have first aid training available for review.

Facility does not have a current fire inspection.

Based on the information obtained during investigation it was determined that a caregiver grabbed a child inappropriately.
