The playground maintenance checklist is not being completed.

The operation made a change in space that made the playground unusable.

The director was lacking training in the required topic of child maltreatment.

A caregiver ws observed changing a child's diaper on their lap. This was corrected at inspection when the caregiver began to change diapers on an appropriate diaper changing table. It was also addressed with the caregiver.

Documentation for safety drills was not available for review at the time of inspection.

Three out of four personnel records evaluated were lacking multiple training certificates.

Two out of four children's records evaluated had out of date immunization records.

Diaper changing supplies were observed to be on and underneath a bench in the main classroom. The diapering supplies were accessible to children in care.

A child was observed having their diaper changed and the child's hands were not washed prior to being laid down for nap. This was corrected when it was addressed with the caregiver and the caregiver washed chilren's hands after diapering.

A caregiver was heard using negative methods of discipline with children in care. The caregiver raised their voice in a negative, harsh tone multiple times. They also utilized negative statements instead of attempting to use positive methods of discipline and guidance.

The operation was lacking an up to date menu. The last menu posted was from April 2025.

The children in care did not wash hands prior to eating lunch. This was corrected at inspection when it was addressed with the caregiver.

Multiple children were observed in a chair with a tray and safety belt for over 15 minutes after they finished eating. They remained in the chair for approximately 1 hour and 45 minutes. This was corrected when the children were removed from the restrictive device.

Two infants under 18 months old were observed to be combined in a classroom with multiple children ranging in age with the oldest child being 11 years old. There were more than 12 children in care at the operation.

There was a 12 year old related to a caregiver laying down in the kitchen area.

There were two uninstalled air conditioning window units in the playground area. The playground also contained a chandelier, rusted items such as a chair and lawn equipment, and there was a pile of wood with rusted nails sticking out. There was also a hammer sitting in the middle of play equipment. There was a broken slide with a puddle of accumluated water on it. Play equipment had a lot of dirt and spiders webs on them. There was also a large spider observed with a spiderweb in the playground area. There was a fallen tree limb inside of the fencing of the playground area. There was siding on the building that appears to be damaged and has nails sticking out. This siding was located in the area fenced for the playground.

Employee records were not available for review at the time of inspection.

A caregiver was observed stepping out of their designated classroom multiple times for tasks such as preparing a meal and checking on another classroom.

The annual certified consumer product safety had not been signed since 2022.

Infant feeding instructions were not updated within 30 days.

3 out of 4 children's records reviewed were lacking a valid health professional statement.

4 out of 4 children's records were lacking admission information such as: address for emergency contact, parent name, physician or hospital information for emergency medical attention, school information, and admission date.

A 7 month old and a 12 month old were combined in a classroom with multiple children ranging in age from 5 years old to 12 years old. There were more than 12 children in care at the operation.

2 out of 4 children's records reviewed had out of date or missing immunization records.

The center was not operating in compliance with multiple minimum standards. Multiple required postings were either out of date or missing. There was not an adequate amount of staff for the center to operate in compliance with standards.

There were a total of 6 infants in care. This exceeds the amount of infants allowed

The operation failed to submit the number of employees that have left their employment in 2023 by January 15th.

The infant classroom had children combined with more than an 18 month old gap in age.

One unqualified caregiver was observed to be in charge of six children.

Infants were in a mixed group of children, a learning video was playing on the television. CI: This was corrected at inspection when the television was turned off.

One out of four staff files evaluated lacked a copy of the photo ID.

One out of four children's records evaluated lacked an immunization record on file.

Two children age 12 months old was mixed with children up to age 12 years old, with 16 total children in care. This was corrected when the infant caregiver arrived and separated the infants from the other children.

The wooden playhouse lacked maintenance; spider webs, leaves and wasp nest.

One fan in the infant room was not mounted. This was corrected at inspection when the fan was removed from the infant room.

The following conditions placed on the operation were not in compliance: Condition 4: The infant classroom checks was not conducted twice a day (morning and afternoon). Condition 7: The indoor available for review.

Operation lacked a fire drill for the month of January.

A five month old infant was placed in the crib on her stomach.

Recited: Seven children were observed in care, the youngest was 3 months old and oldest is three years old.There were more than an 18 month in age difference.

Screen time was observed to be used with three children under the age of two years old.

Two caregivers was responsible for supervising a group of children in care before receiving an eligibility letter.

I observed 22 children in care when the operation's capacity is 20. The operation is over the capacity allowed on the permit.

I observed 22 children in care, where the youngest was 3 months old and oldest is 13 years old, the second youngest child was 2 years old, which is more than 18 months older than the youngest child is allowed.

I observed a 3 month old infant that was awake in a restricted device for 23 minutes. The child was removed by the caregiver.

The Director and an unqualified transportation employee did not have current transportation training.

The operation failed to notify licensing that transportation was being added as a provided service.

The two vehicles that were used for transportation did not have the a list of childrens names and their emergency authorization forms.

The back classroom labeled "B" was being used for storage. part of the storage was removed room was still not usable by children.

An infant was sleeping on a mattress on the floor.A copy of printed TA form restrictive infant equipment was left at the center.

A 7 month old infant was allowed sleep on a mattress with a bottle proped with a pillow. This was corrected at inspection when the pillow and bottle was removed.

A non educational program was observed to be watched by children and several children were playing on hand held video games.. This was corrected at inspection when the hand held games were removed and the television was turned off.
