One of the transportation vans that has been used for transportation does not have a child safe alarm installed.

The cribs in the infant room were not labeled with the child's name or any other identifier indicating which crib belonged to which child. Corrected at inspection: The cribs were labeled with the child's name.

A slat on the privacy fence in the playground was broken at the bottom creating a sharp edge. The texture on the wall in the infant room had begun to crack and come apart. Due to the position of the cribs the texture could fall into the cribs.

The center currently does not have a qualified director designated at the operation.
