One of our six children's files reviewed is missing immunization records.

Two infant age children were asleep in rockers. One of them was covered with a blanket.

There were 6 children present in the infant room and 5 play-yards set up in the room. Play-yards are labeled by numbers. Play-yard #1 was being used by a child not matching the list posted.

Three out of four staff files reviewed are missing information including: -Annual Training -Pre-Service Training

Three out of four staff training records reviewed have expired or are missing Pediatric First Aid / CPR Training.

Last fire inspection was conducted on 8/12/2021.

Infant Feeding Instructions for four children not yet eating table food were not available for review.

Four out of the four files requested for review were not available at the time of inspection.

One caregiver was providing care for five children ages 14mo, 9mo, 5mo, 3mo and 2mo.

Per Daily Schedule infant-age children participate in outdoor play. Infant children were not observed participacing in outdoor play during inspection.

AC vent in the kitchen has build up dust, three baby roaches were observed on the kitchen cabinet and there is garbage in the play-ground. Two expired mayonnaises were observed in the kitchen.

An employee was present at the operation and caring for children without prior background check eligibility notice.

Fire Inspection Report dated 8/12/2021 requested for fire alarm to be tagged by Fire Protech. Most recent tag in panel box is dated 5/18/2021.

Fire extinguishers are on the kitchen floor and not mounted.

Three out of five staff training records reviewed are missing pediatric first aid and CPR training.

Last fire drill, CO detector and fire extinguisher checks were conducted and documented on 1/10/2022.

Play-yards in the infant room are labeled by numbers, however, the assignment list posted is blank. Children nap in solid colors mats, not all mats are labeled with the children's names or assigned numbers while some are labeled but have diferentiate names. There is not a system to differentiate the floor from the child side.

A nine month of old child was observed asleep in play-yard with a blanket. A two month old child was observed asleep in a rocker with a blanket.

There is a crawl and climb caterpillar in the playground. There is no surfacing surrounding the structure nor required use zone space.

Emergency Preparedness Plan is missing lock down and severe weather procedures. Emergency Evacuation and Relocation Diagrams posted do not have the two exit routes required. Diagrams in the toddler and prek room are not posted within children's view.

A caregiver is missing 15 annual training hours. A second caregiver is missing 13 annual training hours.

A caregiver file is missing proof of education.

A caregiver does not have first aid training.

A child's file is missing a Health Care Statement.
