During the investigation, it was found that a staff member did hit a child in the mouth.

Based on the information obtained throughout the investigation, it was determined that the facility disregarded the need of the children for services by excluding them from attending daycare by asking parent to keep them at home and bring them back after an evaluation being done at the facility due to the oldest child?s behavior.

Based on the information gathered, a 3 year old child was left unsupervised in the playground by the assigned caregiver approximately 8 minutes after play time was over. Child was unharmed.

Based on the information gathered a caregiver did scream in a humiliating manner to a child in her class and would scream to the children in the class several times.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

Two staff member are missing annual training in the following topics: Recognizing and preventing shaken baby syndrome and abusive head trauma; understanding and using safe sleep practices and preventing sudden infant death syndrome(SIDS).
