The safety alarm in all vehicle were not properly working.

Caregivers in two room visited were unable to provide accurate information of the children they were responsible for, information as the exact age of the children was not accurate, this will ensure the caregivers are aware of the ratio they are responsible and the level of supervision that should be provide.

Caregiver allowed a group of children to run around the room, as a result a child was severely injured.

A caregiver did not demonstrate good judgement and competency when allowed children to run around the room and not immediately assisting a child who was injured.

Based on a video footage reviewed, infants were fed while on a pillow and not hold by a caregiver.

A caregiver did not conduct a name to face check during a transition, as a result a child was left unsupervised in a room.

It was observed children running down the hallway during a transition, two children went inside a room without supervision.

At the time of ispection there were 2 children with diagnosed food allergies who did not have Emergency Food Allery Plans in their file. 1 child with a diagnosed food allergy had an incomplete FARE form not signed by the parent.

It was observed a caregiver using her personal laptop while supervising children during nap time. Corrected during inspection.

This standard was evaluated in regards to an abuse/neglect investigation. The director did not ensure that the center operated in compliance with the Minimum Standard Rules. This is evidenced by a caregiver being allowed to remain in the classroom after becoming aware of allegations of inappropriate touching and behavior by a caregiver. The director is no longer employed at the operation.

This standard was evaluated in regards to an abuse investigation. At least two caregivers witnessed questionable sexually oriented behavior by a caregiver and failed to report the incidents.

This standard was evaluated in regards to an abuse investigation. Concerns regarding a caregiver's inappropriate touching and questionable behavior towards children in care were not reported by the director or by upper management.

This standard was evaluated in regards to a report and found deficient. It was determined a caregiver was rough with the children while discipling them and was not interacting with them in a positive manner.

Based on the investigation findings, this standard was reviewed and found to be in non-compliance. The child was left unsupervised in their classroom for 16 minutes.

A caregiver with access to children does not have background check results on file. They left the operation prior to end of inspection.
