Napping mats are labeled by numbers but assignment list was not available for review at the time of inspection. Note: This was corrected at inspection when provider generated an assignment list.

Broken equipment and trash were observed in the playground. Note: This was corrected at inspection when provider removed the trash and broken equipment.

Operational policies were reviewed and are missing the following information: - 18(f) Criteria to determine extreme weather for outdoor play; - 18(g) Plan for outdoor play during extreme weather; - 20 Parent's rights; - 26abcde Abuse / Neglect policy

Children in the infant room were served milk, chicken nuggest and beans for lunch. No fruit or second/ additonal vegetables were served.

Most recent letter or form from CCR inspection or investigation was not posted. Updated menu reflecting the food items served during today's meal was not posted.

One infant milk bottle and one infant water bottle were not labeled with the children's names. Note: this was corrected at inspection when provider labeled the bottles with a marker.

Up to date written lesson plans were not available for review nor posted at the time of inspection.

Six staff files were reviewed. One file has an invalid high school diploma. A second file is missing 3 hr of management training. A thrid file is missing notarized affidavit (2895), pre-employment affidavit (2912) and TB test. A fourth file is misising ID, notarized affidavit (2895), pre-employment affidavit (2912) and TB test.

Ten children's files were reviewed. One file is missing up date feeding instructions and emergency medical authoriazation. A second tile is missing custody court order.

Based on the information obtained during the course of the investigation, operation failed to obtain updated signed and dated enrollment agreements after operational policies were updated on 05/06/2024.

Based on the information obtained during the investigation, it was determined that a caregiver did not use gloves when providing first-aid to a child who was injured in care.

A child's incident report was not available for review during the inspection.

Based on the information obtained during the investigation, it was determined the operation did not notify Child Care Regulation of an injury to a child in care that required medical treatment by a health-care professional.

A turtle sandbox was filled with standing water in the children's outdoor playground. Note: This was CORRECTED during the inspection as the sandbox was drained.

Two infants were combined with older children during naptime in a preschool room. There was more than an 18 month difference between the youngest and oldest child in the group. The youngest child was 16 months of age and the oldest child in the group was 5 years of age.

Trash was observed in the outdoor playground. Several candy wrappers and a torn foam cup were observed on the ground accessible to children in care.

A children's toilet in a afterschool room was missing the toilet seat. A second toilet in a different afterschool room was missing the toilet handle.

An individual (new employee) without an active background check was observed inside a classroom having contact with children in care. Individual was not observed alone with the children. NOTE: This was corrected during the inspection. Individual left the operation during the inspection.

Children's products certification was not completed timely. The last completed certification dates 7/13/2022.

Training certificates for these six (6) topics were not available for review for 1 out of 5 caregiver records reviewed.

Food was observed uncovered in the refrigerator. Food uncovered included a cut watermelon, shrimp and cheese, and a small bowl of liquefied food. NOTE: This was corrected during the inspection. Food was removed and disposed of.

The toilet seat was missing from a toilet in one of the afterschool restrooms.

There were no interest centers in the afterschool room.

There was no food allergy emergency plan available for a child with a known food allergy.

1 of 4 children's files reviewed was still missing the child's immunization record. This standard is being RECITED.

The emergency medical transport and treatment authorization forms for14 children being transported were not available in the transportation vehicle folder. Note: This was CORRECTED during the inspection as the required forms were obtained and filed in the transportation's vehicle folder.

The director and a caregiver did not have current training in pediatric cardiopulmonary resuscitation (CPR).

The floor mats where two children were napping on were observed to be torn. Note: The floor mats were switched to cots for both of the children during the inspection.

The Childrens Product Certification form was not current. The date on the form was 7/20/2020.

A container of food was observed inside the refrigerator without a cover. Note: This was CORRECTED during the inspection as the container of food was covered.

The diaper changing mat in the infant room was torn.

3 of 4 children's files reviewed did not have a current immunization record.

The director and a caregiver did not have current training in pediatric first aid with rescue breathing and choking.

The children's outdoor playground area was not maintained. A utility scaffold used for painting was observed within children's reach. This posed a safety hazard as children may climb the equipment and accidentally fall. Grass stickers and animal droppings were also observed in the outdoor playground accessible to children. The paint on the concrete wall was peeling and accessible to children. Note: The equipment used for painting, the animal droppings, and the grass stickers were removed from the outdoor playground during the inspection.

The First Aid Kit in one of two transportation vehicles was not complete as it was missing tweezers. The thermometer in the First Aid Kit for both transportation vehicles was not working. Note: This was CORRECTED during the inspection as the missing equipment was obtained.
