There was sufficient evidence found during the DFPS investigation to show that caregivers did not provide adequate supervision when one child was found unattended outside of the daycare.

There was sufficient evidence found during the DFPS investigation to show that one classroom was out of child/caregiver ratio on the playground.

During the inspection, one caregiver did not wash an infants hands after a diaper change.

During the inspection, one caregiver stated that they were responsible for 6 infants. There were 7 infants observed in the classroom.

One child was observed to be seated in a highchair without the safety straps buckled. The child was removed from the chair and seated for lunch.

A cabinet door was observed to be broken with a nail exposed. There was also an air vent observed to be warped and in disrepair.

It was observed that a classroom of 18 children aged two years old were made to sit at tables for an extended period of time while their meals were being prepared by a caregiver.

During the investigation, it was found that caregivers were preparing meals for one classroom of children when one child was injured.

The required postings were observed to be in an area of the operation that is not easily viewable by staff or parents.
