During the DFPS investigation, there was sufficient evidence found to show that a caregiver used inappropriate discipline when the caregiver dragged a child by the arm.

The infant daily reports did not have sections for each infant's mood and a summary of daily activities.

Observed tall shelves that were easy to tip over that were not mounted in the school age area. I spoke to the operation about mounting the shelves due to tipping. There was a metal rebar sticking out of the playground near the fence. The rebar was removed from the playground.
