The CPR training received by three caregivers did not meet the required AHA guidelines.

An adult chair in the infant classroom had torn upholstery, exposing the foam. This was corrected when the chair was removed from the classroom

Two classrooms were observed with inadequate lighting making it difficult to see children's faces.

During a DFPS investigation, there was sufficient evidence to support that a caregiver neglectfully supervised a child.

During an investigation inspection it was found that the operation is using an unsanitary environment to wash dishes.

2 out of 12 caregivers were missing the trainings required for HASA.

It was found during the inspection the operation had crib sheets that were loose.

It was found during the investigation a caregiver intentionally hit a child on the head with a folded blanket.

It was found during the investigation nap mats were observed to be touching each other during naptime with no space in between.

It was found during the investigation a caregiver lacked good judgment when they covered children's faces with blankets and used one blanket to cover three children.

It was found during the investigation caregivers would leave the nap room unsupervised to use the restroom or to grab items from outside the classroom.

3 of 8 personnel records reviewed were missing communicable diseases and hazardous materials training. 4 of 8 personnel records reviewed were missing physical premises safety, emergency preparedness, communicable diseases and hazardous materials training. 1 of 8 personelle records reviewed were missing food allergy, physical premises safety, emergency preparedness, communicable diseases and hazardous materials training.

3 infants, two in cribs ages 10 months and 11 months and one on a mat on the floor age 11 months, had their heads completely covered with blankets.

A crib had a boppy pillow in it with an infant that was 10 months old.

During an investigation it was found that a caregiver used prohibited punishment, in the form of spanking, on their own child while the group was returning from a walking field trip at the park.

The operation did not self report a discipline concern brought to their attention by a concerned neighbor.

During an investigation, it was found that the operation is not always recording all instances of a child receiving medication.

During an investigation, it was found that the operation failed to report injury to a child where the child sought medical attention.

During an investigation it was found that the operation did not obtain written authorization, from the parent, to give the medication to the child daily. According to the operations policies written authorization is required to administer medication to children when in care.

During an investigation, it was found that a child was being administered prescription medication, with no label or prescription and no box.

3 of 8 personnel records reviewed did not have a notarized affidavit.

A child with a severe food allergy did not have a completed food allergy plan on file.
